Food Vendor Application Form
Thank you for your interest in participating in Out on fifth!
The Oregon District Business Association wants food vendors to draw customers while maintaining all Public Health Department guidelines as well as all food handling safety precautions.
If selected, you will be contacted by the Project Coordinator within 7 to 14 days of form completion to discuss dates, timeframes and details. Submission of an application does not guarantee space will be secured, as vehicular traffic is not permitted on the street during Out on Fifth (unless planned and approved by the ODBA).
Upon approval the Project Coordinator will collect a non-refundable Entry Fee of $150.00. This Fee is due before or upon arrival.
You will be responsible for providing all equipment necessary to offer your product. You will be responsible for all set up and teardown within your timeframe.
The Oregon District Business Association or any business you are set up near will not be held responsible for any loss or damage to your equipment or product.
As pertinent the Project Coordinator will communicate any weather related delays or cancelations.

Food that is prepared on site
In situations where food is being handled or where Time/Temperature controls are necessary for food safety the Vendor must have one of the following License or Permit.
Food Service Operation License
Retail Food Establishment License
Temporary Permit provided by the Montgomery County Department of Health.
Prepackaged, labeled food items for sale
All food product must be packaged and properly labeled for sale. Each label must contain the following information.
Product name
Business contact; Name, address of the person/company responsible for preparation and packaging
Product weight
Ingredients listed clearly
