Out on 5th Entertainment and Pop-up Application Form

Thank you for your interest in participating in The Oregon District's Out on 5th Program.

The Oregon District Business Association wants Out on 5th entertainment and retail pop-ups to draw customers while maintaining all Public Health Department guidelines and operating within City Noise Ordinances.

NO vehicular traffic is allowed on 5th street during the Out on 5th event. This includes: Food trucks, Mobile boutiques, vehicles of any kind.

If selected, you will be contacted by the Project Coordinator within 7 to 14 days of form completion to discuss dates, timeframes and details.  Submission of an application does not guarantee a performance or retail space will be secured.

Upon approval the Project Coordinator will collect a Non-refundable Entry Fee of $15.00 for Performance Groups and $75.00 for Visiting Vendors.
This Fee is due before or upon arrival.

While the overall timeframe of Out on 5th is 11:00am to 10:00pm the Project Coordinator will work with you on a specific timeframe for your performance/pop-up.

You will be responsible for providing all equipment necessary for your performance/Pop-up. You will be responsible for all set up and teardown within your timeframe.   The Oregon District Business Association or any business you are performing or set up near will not be held responsible for any loss or damage to your equipment or product.

INSURANCE: Acts which may be considered dangerous must provide insurance. The licensee shall maintain a policy which limits liability for bodily injury and property damage for personal injury to any person(s), and for damages to property resulting from my appointed time for Out on 5th. The licensee must provide proof of such insurance to the project coordinator or the Oregon District Association upon request.

As pertinent the Project Coordinator will be responsible for communicating any weather related delays or cancelations to Entertainers and Retailers
